EFFECT OF MICROBIAL CELL PREPARATION ON RENAL PROFILE AND LIVER FUNCTION AMONG TYPE 2 DIABETICS: A RANDOMIZED CONTROLLED TRIAL
DESCRIPTION : The beneficial effect of probiotics on renal profile and liver function has been reported among patients with chronic kidney disease and fatty liver respectively. However, its effect on renal profile and liver function among type 2 diabetic individuals has not been fully understood. To investigate the effect of microbial cell preparation on renal profile and liver function tests among type 2 diabetic individuals. Methods A randomized, double-blind, parallel-group, controlled clinical trial was conducted on a total of 136 type 2 diabetics age 30-70 years old in a teaching hospital in Kuala Lumpur, Malaysia. Subjects were randomly assigned to receive microbial cell preparation (N = 68) or a placebo (N = 68) for 12 weeks. The outcomes measured at baseline, week 6, and week 12 and included changes in renal profile (Sodium, Potassium, Urea, Creatinine, Glomerular Filtration Rate), and liver function tests (Albumin, Total Protein, Alkaline Phosphatase, Alanine Aminotransferase, Aspartate Aminotransferase). Intention to treat (ITT) analysis was performed on all the recruited subjects, while per protocol (PP) analysis was conducted on those who completed the trial with good compliance. Result The urea levels significantly declined in the probiotic group. Serum urea levels reduced from 4.26 mmol/L to 4.04 mmol/L in Probiotic Group while it increased in Placebo Group from 4.03 mmol/L to 4.24 mmol/L. These changes were significant between groups in ITT analysis (p = 0.018). Other parameters did not change significantly between groups. Conclusion 12 weeks supplementation with daily dosage of 6 × 1010 Colony Forming Units of multi-strain microbial cell preparation significantly improved urea levels.

EFFECT OF DIFFERENT DRYING TREATMENTS AND SOLVENT RATIOS ON PHYTOCHEMICAL CONSTITUENTS OF IPOMOEA AQUATICA AND CORRELATION WITH α-GLUCOSIDASE INHIBITORY ACTIVITY
DESCRIPTION : Ipomoea aquatica is an aquatic plant that is widely consumed in Southeast Asia as a vegetable. In this study, the influence of various ethanol ratios (0, 20, 50, 80, and 100%) as an extraction solvent and different drying methods including air drying (AD), sun drying (SD) and oven drying (OD) on phytochemical constituents of I. aquatica was investigated using a proton nuclear magnetic resonance (1H NMR) based metabolomics approach. The effect on α-glucosidase inhibitory activity and total phenolic content (TPC) was also examined. Clear discrimination was observed between different ethanol ratios and different drying processes by principal component analysis (PCA). The highest α-glucosidase inhibitory activity was observed for absolute ethanol extract from the OD method with IC50 value of 204.0 ± 59.0 µg/mL and TPC value of 22.0 ± 0.7 µg GAE/mg extract. Correlation between the α-glucosidase inhibitory activity and the metabolite were analyzed using a partial least square (PLS) analysis. The metabolites that are responsible for the activity were quercetin derivatives, chlorogenic acid derivatives, sucrose and fructose. This study highlights the basis for future investigations of I. aquatica as a source of food that has the potential for nutraceutical enhancement and as ingredient in medicinal preparation.

URINARY METABOLOMICS STUDY ON THE PROTECTIVE ROLE OF ORTHOSIPHON STAMINEUS IN STREPTOZOTOCIN INDUCED DIABETES MELLITUS IN RATS VIA 1H NMR SPECTROSCOPY
DESCRIPTION : Orthosiphon stamineus (OS) is a herb known in ethnomedicine for treating diabetes mellitus (DM). In this study, a 1H NMR based urine metabolomics tool has been used for the first time to identify the metabolic protective mechanism of OS in DM using Streptozotocin (STZ) induced experimental model in rats. Methods Four different solvent extracts of OS, namely aqueous, ethanolic, 50% aqueous ethanolic and methanolic, at a dose of 500Â mg/kg body weight (bw) were orally administered for 14 days to diabetic rats induced via intraperitoneal injection of 60Â mg/kg bw STZ. NMR metabolomics approach using pattern recognition combined with multivariate statistical analysis was applied in the rat urine to study the resulted metabolic perturbations. Results OS aqueous extract (OSAE) caused a reversal of DM comparable to that of 10Â mg/kg bw glibenclamide. A total of 15 urinary metabolites, which levels changed significantly upon treatment were identified as the biomarkers of OSAE in diabetes. A systematic metabolic pathways analysis identified that OSAE contributed to the antidiabetic activity mainly through regulating the tricarboxylic acid cycle, glycolysis/gluconeogenesis, lipid and amino acid metabolism. Conclusions The results of this study validated the ethnopharmacological use of OS in diabetes and unveiled the biochemical and metabolic mechanisms involved.

IN-VIVO STUDY OF SYNTHETIC AND NATURAL FOOD COLORS EFFECT ON BIOCHEMICAL AND IMMUNITY PARAMETERS
DESCRIPTION : Incorporation of the coloring agent, tartrazine and curcumin affects healthy physiological system leading to changes in hematology, biochemistry and enzymatic activity. Consequences of oral administration of tartrazine and curcumin were explored for 15, 30 and 45 days in female adult rat. Two doses were based on the admissible daily intake (ADI) of 9.6 and 96 (high) mg/kg/body weight for tartrazine, 3.85 and 38.5 mg/kg/body weight for curcumin. Phytochemicals such as saponins, glycosides, alkaloids, flavonoids, tannin, carbohydrates and phenolics were present in coloring agents. The renal function tests and lipid profile showed alterations in the values of uric acid, urea, total protein, albumin and cholesterol in all treatment groups. Additionally, the levels of liver enzymes fluctuated after 45 days of treatment with curcumin and tartrazine. Histopathological changes were also recorded in liver and kidney. In contrast, blood biochemistry depicted the significant reduction of RBCs, Hb, MCH, glucose, WBCs, MCHC and LDH at 15, 30 and 45 days. Besides, IgG, IgM and overall antibody GMT at 45 days were significantly increased. Study indicated that ADI and doses up to 10 times of the ADI of food colors intake exerted adverse effects on immune response and altered the biochemical parameters.

IN DEPTH ANALYSIS OF THE SOX4 GENE LOCUS THAT CONSISTS OF SENSE AND NATURAL ANTISENSE TRANSCRIPTS
DESCRIPTION : SRY (Sex Determining Region Y)-Box 4 or Sox4 is an important regulator of the pan-neuronal gene expression during post-mitotic cell differentiation within the mammalian brain. Sox4 gene locus has been previously characterized with multiple sense and overlapping natural antisense transcripts [1], [2]. Here we provide accompanying data on various analyses performed and described in Ling et al. [2]. The data include a detail description of various features found at Sox4 gene locus, additional experimental data derived from RNA-Fluorescence in situ Hybridization (RNA-FISH), Western blotting, strand-specific reverse-transcription quantitative polymerase chain reaction (RT-qPCR), gain-of-function and in situ hybridization (ISH) experiments. All the additional data provided here support the existence of an endogenous small interfering- or PIWI interacting-like small RNA known as Sox4_sir3, which origin was found within the overlapping region consisting of a sense and a natural antisense transcript known as Sox4ot1.

PERCEPTION OF MALAYSIAN HEALTHCARE PROFESSIONAL TOWARDS FAMILY MEDICINE SPECIALISTS
DESCRIPTION : Perception of healthcare providers who worked with family medicine specialists (FMSs) could translate into the effectiveness of primary healthcare delivery in daily practices. This study examined perceptions of public healthcare providers/professionals (PHCPs) on FMSs at public health clinics throughout Malaysia. This was a cross-sectional study in 2012-2013 using postal method targeting PHCPs from three categories of health facilities, namely health clinics, health offices and hospitals. A structured questionnaire was developed to assess PHCP's perception of FMS's clinical competency, safety practice, ethical and professional values, and research involvement. It consists of 37 items with Likert scale of strongly disagree (a score of 1) to strongly agree (a score of 5). Interaction and independent effect of the independent variables were tested and adjusted means score were reported. The participants' response rate was 58.0% (780/1345) with almost equal proportion from each of the three public healthcare facilities. There were more positive perceptions than negative among the PHCPs. FMSs were perceived to provide effective and safe treatment to their patients equally disregards of patient's social background. However, there were some concerns of FMSs not doing home visits, not seeing walk-in patients, had long appointment time, not active in scientific research, writing and publication. There were significant differences in perception based on a respondent's health care facility (p < 0.0001) and frequency of encounter (p < 0.0001). PHCPs had overall positive perceptions on FMSs across all the domains investigated. PHCPs from different health care facilities and frequency of encounter with FMSs had different perception. Practicing FMSs could improve on the critical service areas that were perceived to be important but lacking. FMSs might need further support in conducting research and writing for publication.

THE MALAYSIAN GESTATIONAL DIABETES AND PREVENTION OF DIABTES STUDY (MY GODDESS)
DESCRIPTION : Over 50% of women with a history of gestational diabetes mellitus (GDM) will develop type 2 diabetes (T2D) in later life. Asian women experience a disproportionate risk of both GDM and T2D compared to women from other ethnic backgrounds. Lifestyle interventions and behavior change can delay or even prevent the onset of T2D. We have developed a digitalized diabetes prevention intervention for the prevention of T2D in Malaysian women with GDM.

A SYSTEMATIC REVIEW OF THE EPIDEMIOLOGY OF HEPATITIS E VIRUS INFECTION IN SOUTH – EASTERN ASIA
DESCRIPTION : Hepatitis E virus (HEV) infection is an emerging zoonotic viral disease, with an increasingly international public health challenge. Despite the concerns that the global disease burden may be underestimated. Therefore, evaluation of the disease epidemiology in South – eastern Asia through a systematic review will assist in unraveling the burden of the disease in the subregion. A priori protocol was prepared for the systematic review and followed by a literature search involving five electronic databases. Identified publications were screened for high quality studies and the elimination of bias and relevant data extracted. A total of 4157 citations were captured, and only 35 were included in the review. A wide range of HEV seroprevalence was recorded from 2% (urban blood donors in Malaysia) to 77.7% (lowland communities in Lao PDR). Sporadic HEV infection and epidemics were also detected in the subregion. Indicating hyperendemicity of the disease in South – eastern Asia

HUMAN MESENCHYMAL STEM CELLS-MEDIATED TRANSCRIPTOMIC REGULATION OF LEUKEMIC CELLS IN DELIVERING ANTI-TUMORIGENIC EFFECTS
DESCRIPTION : Treatment of leukemia has become much difficult because of resistance to the existing anticancer therapies. This has thus expedited the search for alternativ therapies, and one of these is the exploitation of mesenchymal stem cells (MSCs) towards control of tumor cells. The present study investigated the effect of human umbilical cord-derived MSCs (UC-MSCs) on the proliferation of leukemic cells and gauged the transcriptomic modulation and the signaling pathways potentially affected by UC-MSCs. The inhibition of growth of leukemic tumor cell lines was assessed by proliferation assays, apoptosis and cell cycle analysis. BV173 and HL-60 cells were further analyzed using microarray gene expression profiling. The microarray results were validated by RT-qPCR and western blot assay for the corresponding expression of genes and proteins. The UC-MSCs attenuated leukemic cell viability and proliferation in a dose-dependent manner without inducing apoptosis. Cell cycle analysis revealed that the growth of tumor cells was arrested at the G0/G1 phase. The microarray results identified that HL-60 and BV173 share 35 differentially expressed genes (DEGs) (same expression direction) in the presence of UC-MSCs. In silico analysis of these selected DEGs indicated a significant influence in the cell cycle and cell cycle-related biological processes and signaling pathways. Among these, the expression of DBF4, MDM2, CCNE2, CDK6, CDKN1A, and CDKN2A was implicated in six different signaling pathways that play a pivotal role in the anti-tumorigenic activity exerted by UC-MSCs. The UC-MSCs perturbate the cell cycle process of leukemic cells via dysregulation of tumor suppressor and oncogene expression.

DATA FROM: MOBILE PHONE REMINDERS AND PEER COUNSELING IMPROVE ADHERENCE AND TREATMENT OUTCOMES OF PATIENTS ON ART IN MALAYSIA: A RANDOMIZED CLINICAL TRIAL
DESCRIPTION : Background: Adherence to treatment remains the cornerstone of long term viral suppression and successful treatment outcomes among patients receiving Antiretroviral Therapy (ART). Objective(s): Evaluate the effectiveness of mobile phone reminders and peer counseling in improving adherence and treatment outcomes among HIV positive patients on ART in Malaysia. Methods: A single-blind, parallel group RCT conducted in Hospital Sungai Buloh, Malaysia in which 242 adult Malaysian patients were randomized to intervention or control groups. Intervention consisted of a reminder module delivered through SMS and telephone call reminders by trained research assistants for 24 consecutive weeks (starting from date of ART initiation), in addition to adherence counseling at every clinic visit. The length of intended follow up for each patient was 6 months. Data on adherence behavior of patients was collected using specialized, pre-validated Adult AIDS Clinical Trial Group (AACTG) adherence questionnaires. Data on weight, clinical symptoms, CD4 count and viral load tests were also collected. Data was analyzed using SPSS version 22 and R software. Repeated measures ANOVA, Friedman's ANOVA and Multivariate regression models were used to evaluate efficacy of the intervention. Results: The response rate after 6 months follow up was 93%. There were no significant differences at baseline in gender, employment status, income distribution and residential location of respondents between the intervention and control group. After 6 months follow up, the mean adherence was significantly higher in the intervention group (95.7; 95% CI: 94.39–96.97) as compared to the control group (87.5; 95% CI: 86.14–88.81). The proportion of respondents who had Good (>95%) adherence was significantly higher in the intervention group (92.2%) compared to the control group (54.6%). A significantly lower frequency in missed appointments (14.0% vs 35.5%) (p = 0.001), lower viral load (p = 0.001), higher rise in CD4 count (p = 0.017), lower incidence of tuberculosis (p = 0.001) and OIs (p = 0.001) at 6 months follow up, was observed among patients in the intervention group. Conclusion: Mobile phone reminders (SMS and telephone call reminders) and peer counseling are effective in improving adherence and treatment outcomes among HIV positive patients on ART in Malaysia. These findings may be of potential benefit for collaborative adherence planning between patients and health care providers at ART commencement.

IAT DATA
DESCRIPTION : A cross sectional study that was conducted among undergraduate students from June 2020 until October 2020, in a public university in Malaysia. . The IAT questionnaire created by Kimberly Young (1998) was adopted(Young, 1998). This questionnaire has 20 questions with six Likert scale that include the following options from not applicable”, “rarely”, “occasionally”, “frequently”, “often” and “always”. There were four possible methods on calculating the total IAT, namely Method 1, Method 2, Method 3 and Method 4. For Method 1, “not applicable” option was not counted in the total scores of IAT. Only questions with answers from “rarely” until “always” were scored; scored 1 for “rarely” , scored 2 for “occasionally”, scored 3 for “frequently”, scored 4 for “often” and scored 5 for “always”. The total score were summarised and then converted to 100%. For example, if respondent A answered 18 questions, therefore total scores were the summation of 18 questions and then converted to 100%. For Method 2, “not applicable” option was scored as 0 followed with, rarely (scored 1) ,occasionally (scored 2), frequently (scored 3), often (scored 4) and always (scored 5). Total scores were summarisation of 20 questions, with ranging from 0 to 100%(Lu and Yeo, 2015b; Haque et al., 2016; Othman and Lee, 2017; Azhar and Yunus, 2020; Padmavathy KM, Hakim AR, Kalisyah A, Yuhanis Y, Irfan ZA, Luqman R, Arif Z, 2020). For Method 3, six Likert scale was converted to five Likert scale with assuming those respondents who answered “not applicable” option equals to “rarely”. This is assuming that, if the questionnaire does not have “not applicable” option, respondents might choose “rarely” as their answer. Total scores were ranging from 20 to 100(Zainudin, Din and Othman, 2013; Chong Guan et al., 2015; Ching et al., 2017; Rosliza et al., 2018; Mooi et al., 2019; Ying Ying et al., 2021). For Method 4, those questions that were answered with “not applicable” option were treated as missing data, therefore multiple imputation was used. Total scores were ranging from 0 to 100. Total scores from the four different methods were then categorized to four categories variable (IAT-4 categories), including low IA (0-30 scores), mild IA (31-49 scores), moderate IA (50-79 scores) and severe IA (80-100 scores). Further categorization to two categories (IAT-2 categories) with non-IA (low and mild IA: 0-49) and IA (moderate and severe IA: 50-100) was computed.

SCAN FINDINGS OF ISCHAEMIC HEART DISEASE IN ADULTS AGED 30-40 YEARS OLD AS DETECTED AND EVALUATED ON MYOCARDIAL PERFUSION SCINTIGRAPHY
DESCRIPTION : Introduction: Ischaemic heart disease (IHD) is on the rise among adults under 40-years. Radionuclide myocardial perfusion scintigraphy (MPS) can provide imaging information to diagnose, evaluate and risk stratify IHD. Nevertheless, local data are limited on the usage of this technique in identifying IHD among young adult patients. Our objectives were to determine important clinical characteristics of these patients who underwent MPS for IHD detection and highlight their scintigraphy findings. Methodology: A case-series and pictorial review of adults aged 30-40 years old (n=34) who had two-day protocol MPS using 99mTechnetium-tetrofosmin done between January and December 2019. Scan acquisition being performed as per standard departmental protocol. Reversible perfusion defect signified ischaemia whereas irreversible perfusion defect implied infarction. Clinical information and scan findings were compiled and analysed. Results: Average age was 36.59 years. Females were predominant (n=19, 55.9%). Majority had ≥2 cardiovascular risk factors (n=22, 64.7%). Small proportion of patients had morbid obesity (n=3, 8.8%) and intermediate risk for pre-test IHD probability (n=12, 35.3%). Imaging findings of MPS included normal perfusion (n=9, 26.5%), artifact (n=1, 2.9%), ischemia (n=23, 67.6%) and infarction (n=1, 2.9%). Mean ejection fraction on stress MPS was 66%. Patients with ≥2 cardiovascular risks and intermediate probability were significantly associated with abnormal MPS findings (p<0.05). Conclusions: IHD was observed in 70.5% of adults under 40-years who were referred for MPS. Those who had ≥2 cardiovascular risks and intermediate probability were significantly associated with abnormal MPS findings. Attention should be given during scintigraphy images interpretation and reporting of these particular patients

THE VOLUME OF WASTE GENERATED AND ITS IMPACT ON GHG EMISSIONS, LAND USE, AND HEALTH RISK ASSESSMENT: A STUDY IN MALAYSIA
DESCRIPTION : Waste dumped in landfills was classified as mixed domestic waste or landfilled waste where the available data from 2014 to 2018 only at the time of data collection. Meanwhile, segregated wastes (plastic, paper, metal, aluminium, glass, and e-waste) were collected separately for recycling. As the mandatory waste segregation at source programme started in September 2015, the available data on segregated waste were selected from 2015 to 2018.
